Before you register on line please have a scanned birth certificate in JPG format <1 Mb and a picture of players head only in JPG format <1 Mb, that is ready to be uploaded.

1. Cleveland Select Registration form (US Club Soccer)
   This registration is mandatory for all Cleveland Select Players

3. Please go to shop/merchandise and order your uniform, and any other Club gear that you wish. For the first game you can wear a yellow t-shirt till you get your uniform.
4. CS requires that the player’s soccer ball is INFLATED AT ALL CS activities
